2) ملخّص المقرر ومراميه العامة

2) Crouse abstract & General Goals

يتضمن المقرر:

‌أ)        التوليد الطبيعي : يتضمن لمحة عن العناية بصحة المرأة،المقاربة السريرية للحامل والماخض، لمحة تشريحية وجنينية عن الجهاز التناسلي الانثوي،التبدلات الفيزيولوجية والغدية أثناء الحمل والمخاض، وتكيف الأم من الناحية الفيزيولوجية والمناعية مع الحمل،رعاية الحامل،دراسة مراحل المخاض والولادة في المجيئات والأوضاع النظامية والمعيبة وفي الحمل المتعدد، المراقبة الجنينية ،النفاس الطبيعي،والارضاع.

‌ب)   التوليد المرضي: يتضمن الاختلاطات التي تتعرض لها الحامل والماخض والنفساء كالنزوف التوليدية،الانتان النفاسي ،شذوذات المخاض وعسرات الولادة، تمزق الأغشية الجنينية الباكروالمخاض المبكر،تأخرالنموداخل الرحم ،الحمل المديد،الحمل المتعدد والمجيئات المعيبة،ارتفاع التوتر الشرياني الحملي، تنافر الزمر الدموية  وعامل ريزوس،الإنتانات التي تصيب الحامل، الاختلاطات الداخلية والجراحية المرافقة للحمل،والتوسطات التوليدية مثل القيصرية وملقط الجنين والمحجم السويدي.

‌ج)    التدريب السريري :

‌د)      يتم فيه التركيز على الاستجواب والفحص السريري النسائي والتوليدي،تشخيص واستقصاء وعلاج المشاكل المتعلقة بالحمل والمخاض والنفاس والأمراض النسائية ،التدريب على الولادة الطبيعية ،الاطلاع على العمليات التوليدية ، وتدبير الاختلاطات الطبية والجراحية أثناء الحمل والمخاض والنفاس.

أ‌)        الأهداف :

·       الغايات العامة ‏General Aims: في المرحلة الجامعية يكون التركيز الأساسي على المهارات السريرية والكفاءات المتعلقة بالحمل واختلاطاته

·        اكساب الطالب مهارة مقاربة الحامل والماخض والنفساء ، واكسابه مهارة التشخيص والعلاج،وعند الضرورة الاحالة الى المراكز المختصة .

·       اكساب الطالب مهارة ممارسة الولادة الطبيعية وتدبير الاختلاطات التوليدية الشائعة.

The course includes:

A.            Normal obstetrics: includes a life-course perspective for women’s health care, clinical approach to the patient during pregnancy and labor, female reproductive anatomy and embryology, endocrinology of pregnancy and parturition, maternal physiologic and immunologic adaptation to pregnancy, antepartum care, normal labor, fetal surveillance during labor, normal puerperium, and lactation.

B.            Abnormal obstetrics: It includes complications experienced by pregnant and laboring women such as Obstetric Hemorrhage and Puerperal Sepsis, Uterine Contractility and Dystocia, Obstetric Complications, Multifetal Gestation and Malpresentation, Hypertensive Disorders of Pregnancy, Rhesus Isoimmunization, Common Medical and Surgical Conditions Complicating Pregnancy, and Obstetric Procedures.

C.            Clinical Training:

With a focus on the interrogation and clinical examination of pregnant women, diagnosis and investigation and treatment of problems related to pregnancy, labor and postpartum and gynecology, training in natural childbirth, access to obstetric and women's operations, and management of medical and surgical complications during pregnancy, labor and postpartum.

D.          Objective:

• General Aims: In Undergraduate, be the primary focus on clinical skills and competencies and pregnancy-related complications.

·       providing students with the skill of approach of women during pregnancy, labor and postpartum, and improves their skills in diagnosis and treatment, and, if necessary, referral to the competent centers.

• give the student skill practice natural childbirth and management of women's common obstetric complications.

بعد إنهـاء هذا المقرر بنجـاح، سـيكتسـب الطالب معرفـة وفهمـاً في اساسيات طب التوليد ، ويكون قادراً على أن:

·       يشرح التبدلات التشريحية والفيزيولوجية في جهاز المرأة التناسلي في مختلف مراحل العمر وأثناء الحمل والنفاس.

·       يصف تطور الجنين وملحقاته.

·       يصف تطور الحمل الطبيعي

·       يشرح الالية المرضية ،الوبائيات ،أسباب المرض وكيفية تطوره.

·       يصف التظاهرات السريرية للأمراض ،التشخيص ،الكشف المبكر والاختلاطات .

·       يشخص الحمول المرضية ويحدد عوامل الخطورة العالية.

·       يستعرض الأمراض المرافقة للحمل.

·       يشخص المخاض الطبيعي والشاذ .

·       يستعرض طرق إنهاء الحمل المختلفة.

·       يستعرض الخطوط الأساسية لعلاج اختلاطات الحمل

·       يصنف الحالات المرضية في مجموعات ويقارن بينها.

·       يمارس الولادة الطبيعية والتوسطات التوليدية البسيطة تحت الاشراف.

·       يعرف الوليد الطبيعي

·       يساهم في انقاص الوفيات والمراضة حول الولادة .

·       يصف المبادئ الأخلاقية والقانونية لممارسة التوليد والجوانب الطبية الشرعية والأخطاء.

يضمن الخصوصية والسرية للمريضة .

After finishing this course successfully, the student will gain knowledge and understanding of the fundamentals of Obstetrics, and be able to:

•              Explain the anatomical and physiological changes in the reproductive system of women in various stages of life and during pregnancy and postpartum.

•              Describes the development of the embryo and its accessories.

•              Describes the evolution of normal pregnancy

•              Explains the pathogenesis, epidemiology, and causes of the diseases and their developments.

•              Describes the clinical manifestations of diseases, diagnosis, early detection and complications.

•              Diagnose pathological pregnancies and identifies high-risk factors.

•              Review the accompanying diseases of the loader.

•              Diagnosed normal and abnormal labor.

•              Reviews the different methods of ending pregnancy.

•              Reviews the basic guidelines for the treatment of complications of pregnancy

•              Diagnosed normal and abnormal puerperium.

•              Classifies pathological cases in groups and compares them.

•              Practiced natural childbirth and obstetric procedures under supervision.

•              Known natural newborn.

•              Contributes in perinatal mortality and morbidity reduction.

•              Describes the ethical and legal principles in obstetrics and medical aspects and legal errors.

•              Ensures privacy and confidentiality for the patient.

·       أن يحلل معطيات القصة السريرية والفحص السريري
ونتائج الفحوص المخبرية والشعاعية  ويضع التشخيص السريري الأولي والتشخيص التفريقي بهدف حل المشاكل التوليدية السريرية الأساسية والأكثر شيوعا. 

·       أن يناقش كل حالة توليدية على حدة واستنتاج التدبير الاكثر ملائمة لكل مريضة من بين خيارات متعددة.

·       أن ينظيم المعلومات ويدققها ويقيمها.

·       التعرف على المشكلات الصحية التوليدية في المجتمع وايجاد الحلول المناسبة لها .

·       التخطيط المستقبلي لانقاص نسب الوفيات والمراضة الوالدية والوليدية والكشف المبكر عن السرطان وتنظيم الاسرة.

·       أن يقيم كفاءة المعالجات المختلفة من حيث الكلفة المادية.

·       اتخاذ القرارات المناسبة أثناء ممارسة االتوليد.

·       تطبيق التفكير المستند على الدليل والبرهان.

·       Analyze clinical history and general examination data and the results of laboratory tests to make the initial clinical diagnosis and differential diagnosis in order to solve the basic problems of women.

·       Discuss each case individually and infer the most appropriate measure for each patient from among multiple options.

·       Organize information and audited and evaluated.

·       Identify women's health problems in the community and find appropriate solutions.

·       Future planning to lose mortality rates and maternal and neonatal morbidity, and early detection of cancer and family planning.

·       Evaluate the efficiency of the different treatments in terms of material cost.

·       Take appropriate decisions during the practice.

·       Apply document on evidence and proof thinking.

بعد إنهـاء هذا المقرر بنجـاح، سيكون الطالب قادراً على:

After going through this course, student should be able to:

·       اخذ قصة سريرية مفصلة من الحامل أو الماخض وتسجيل وتوثيق البيانات.

·       إجراء فحص سريري كامل للحامل وتقدير سن الحمل وتدوينه.

·       تشخيص المجيئات الجنينية الطبيعية والمرضية.

·       متابعة الطور الثالث للمخاض.

·       التواصل مع المرضى بطريقة أخلاقية ومهنية.

·       القيام بجميع المهارات العملية الأساسية في التوليد ومنها :

o      وضع قسطرة بولية مفرغة أو دائمة (فولي)عند المرأة.

o      القيام بولادة طبيعية مع توليد المشيمةتحت الإشراف.

o      فحص المشيمة بعد الولادة.

o      المساعدة في اجراء الولادة القيصرية.

·       قراءة نتائج الاستقصاءات المخبرية والشعاعية بشكل صحيح وربطها مع القصة السريرية والفحص السريري .

·       تقديم التدابير الإسعافية الأولية ومهارات الانعاش .

·       وضع خطة تدبير كاملة للمريضة .

·       كتابة وصفة طبية أمنة ومناسبة لجميع الحالات الشائعة.

·       تقديم الرعاية الطبية للحامل .

·       اخذ الموافقة الطبية المستنيرة من المريضة أو ذويها عند الضرورة .

·       تحديد الحالات الواجب إحالتها للمراكز التخصصية .

·       Taking detailed clinical story of a pregnant or labored women, record and document data.

·       Check out the full clinical assessment for pregnant and determines gestational and its codification.

·       Diagnosis normal and abnormal presentation.

·       Follow-up to the third stage of labor.

·       Communicate with patients in an ethical and professional manner.

·       Do all the basic practical skills in obstetrics, including:

o   Put a vicious or permanent urinary catheter (Foley) in women.

o   Do a normal birth under supervision.

o   Do examination of the placenta after birth.

o   Assist in a caesarean section.

·       Read the results of surveys laboratory and radiology properly and link them with clinical history and clinical examination.

·       Provide initial emergency measures and recovery skills.

·       Establish a complete management plan for the patient.

·       Writing a prescription safe and suitable for all common situations.

·       Provide medical care for pregnant women.

·       Taking medical informed consent of the patient or their relatives if necessary.

·       Identification of cases to be forwarded to the specialized centers.
